ABOUT CLINICAL SERVICES

The Florida A&M University, in collaboration with the Florida Department of Health’s Bureau of Chronic Disease Prevention, runs a Telehealth Cardiovascular Disease and Stroke Prevention Program.

The Telehealth Cardiovascular Disease and Stroke Prevention Program is a medication therapy management (MTM) program with the goal of reducing the risk of heart disease and stroke and preventable hospitalization.

Patients with cardiovascular disease deal with multiple chronic medical conditions are on several medications, and often see more than one provider or doctor. Our pharmacist faculty will review your medications and conditions with you and become your advocate to help you receive the best care.

The Telehealth Cardiovascular Disease and Stroke Prevention Program will help all our patients achieve better treatment outcomes through:

  • improving medication adherence,
  • optimizing safety and treatment options for you, and
  • reducing costs associated with unnecessary treatment and preventable hospitalization.

MISSION

The mission of our Telehealth Cardiovascular Disease and Stroke Prevention Program:

  1. Clinical Rotation Site: Our site will serve as a center for clinical rotation. Our learners will develop the expertise to provide medication therapy management (MTM) through hands-on experience.
  2. Clinical Care Outreach: Through a partnership with other health clinics, we will reduce the economic and social impact of cardiovascular disease and stroke by providing MTM services to the medically underserved population.
  3. Residency program: Our center is a home for recently graduated student pharmacists who are interested in a post-graduate year 1 (PGY-1) Pharmacy residency program with a primary focus on primary care and provision of MTM among the medically underserved population - Community Health Northwest Florida. 
  4. Research: The Cardiovascular Disease and Stroke Prevention Program conduct health-outcomes and behavioral research geared towards understanding risk factors and approaches to improve cardiovascular disease and stroke among the medically underserved population. Click here to view a publication list of our studies

  • Homeless
  • Low-income
  • Medicaid-eligible
  • Medicare-eligible
  • Native American, Hispanic or African American
  • Migrant farmworkers
  • Uninsured patients

No, you do not need a referral from your doctor. However, we will communicate with your doctor regarding your care from our service.

We will help you find a doctor who will review our recommendations.

No, there is no cost to you.

No, all of our services are provided either virtually or telephonically.
